Program Overview
The Georgia Planning for Healthy Babies (P4HB) Program is administered by the Department of Community Health (DCH), Division of Medicaid to assist women who deliver low birth weight (LBW) babies to reduce the number of VLWB in the city of Georgia.
The Georgia Planning for Healthy Babies (P4HB) Program provides three types of services to the women who qualify for the waiver. The three components of the P4HB program are,
- Family Planning Only which provides services related to family planning to the families of the participant. The eligible people can avail the benefits until the waiver expires
- Inter-pregnancy Care (IPC) is for women who have delivered a baby who has a meager birth weight. The program also provides family planning services to women.
- Resource Mother Outreach provides case managers who are well-trained to women who gave birth to a VLBW baby.
To provide primary care services to the eligible participants of the Family Planning Only the Georgia Primary Care Association provides a list of referrals that can provide those services.

The women who are eligible for the Georgia Planning for Healthy Babies (P4HB) Program have access to the following services,
- Pregnancy tests
- Family planning lab tests
- Except for Hepatitis B, HIV/AIDS the screening and testing of other STD(s)
- Tubal Ligation (Sterilization)
- Medication and other supplies that are required for the patient during the family planning services
- Various Vitamin Supplements
- Vaccination for Hepatitis B and Tetanus-Diphtheria
- Counseling of patient
- Various social and family planning services
- Contraceptive and other supplies
- Services related to primary health care
Eligibility Requirements
