The “Notice of Daniel” from your social security means that you must have the annual income more than the required amount. And if the annual income is excessive than the requirement. it means you are not eligible for the HoosierRx medicare Program. And in case if the applicant receives the ” Notice of reward ” from the social security it means he is capable of the HoosierRx medicare health Program because his annual income is less than the amount mentioned
5. The applicant should have a medicare part D plan from the companies described below if the applicant doesn’t have this plan from the below companies he will not be capable of having this HoosierRx Program service. The companies are: AARP/United HealthCare

CIGNA, EnvisionRx, First Health, Indiana University Health Plans, SilverScript, WellCare
