These services are for people suffering from developmental disabilities or related disabilities, like autism, intellectual, or substantial physiological disability.

The participant can plan their individual services plan guided by the Individual Support Team.

This team includes:

• The participant
• Case manager

Any person is chosen by the participant ( family members and friends have a preference in this selection)

In this Individualized Support Plan, the participant, with the help of the team, does the following tasks:

• He selects the services of the program
• He identifies and picks the service provider according to his personal preference.
• He develops his plan of care
• In the end, he has been subjected to his annual wavier services cap for the cost of 17,300 dollars.

Family Services Waiver provides you with high profile services that meet the meaningful criteria for the necessary life-based, community-based services, and support. These services fall into the economy of the participants. In these services, one is free to perform his rituals. He can pass his life according to his beliefs. There is discrimination regarding the caste and creed and by the premises or the person. We do our best to provide support in his working environment, and we help the person maintain his social contacts and social standards.

Supervised Group Living

It is an option for the residence of the participant. He may choose a homegroup for the place or go for the Supervised Group Living option. This option is an alternative to the waiver placements for the people who meet these services’ eligibility criteria.

This service is for people who have a disability. However, these disabled people have normal intellectual levels. The State of Indiana has more than 500 Supervised Group Living for needy people. More than 3000 needy are availing this residential service in Indiana. Federal regulations have a corner for these accommodations. The Federal Regulations license and govern these homes. This facility requires recertification on an annual basis.
